Long-Term Condition Reviews

Long Term Conditions (LTC) are health conditions that can impact on a person’s life, and may require ongoing care and support. We support our patients with a variety of Long Term Conditions, including but not limited to:

  • Diabetes
  • Chronic Obstructive Pulmonary Disorder (COPD)
  • Asthma
  • Chronic Heart Disease (CHD)
  • Vascular Conditions
  • Post-Covid

What to expect:

At the time of your diagnosis you will be offered an initial appointment by phone, video consultation, online via MedLink or face to face to discuss your symptoms, self management, any blood test results and medication. Following this most patients will be invited for an annual review, occurring during their birthday month. We invite patients by phone call, text message, and letter. You’ll be seen by an experienced clinician.

What happens during my annual review:

  • You may need to have some tests done before your review, such as a blood test or to do a urine sample
  • Allows one-to-one time to ask a clinician advice about any ongoing problems relating to your condition
  • Your medication is reviewed to ensure it is at optimal level
  • Supports you to learn how to self manage your condition at home inc. advice on alternative therapies

Some patients may require more frequent reviews i.e. 6 monthly. The reasons for this will be discussed with you.

Video Consultations

Video Group Consultations are a way for you to join people with similar health issues and consult with your GP or nurse for longer. We’re now offering Video Group Consultations for our patients with the following long term conditions:

  • Diabetes
  • Chronic Obstructive Pulmonary Disorder (COPD)
  • Asthma
  • Post-Covid

Minor Surgery

Certain minor surgical problems, not requiring a general anaesthetic can be carried out at the surgery. Please see your doctor for assessment so that arrangements can be made.

If you have received an invitation to attend the surgery for annual health check, please read this information before you attend. For an easy read guide to the annual health check please click here.

Please follow this link to a checklist with questions that would be helpful for the doctor or nurse to know before your appointment. Please complete the checklist and email it to [email protected] Alternatively, you may drop a printed copy to your usual surgery before your appointment. If you cannot email a copy or drop a printed copy to the surgery, please print a copy and bring it with you.

Please bring a urine sample, in any clean container, with you when you attend for your health check. This is to check for Diabetes.

It is very important that you have your flu vaccination. If you haven’t already had your flu vaccination you can have this during your health check.

End of Life Care

The Practice adheres to the NHS Gold Standards Framework for end of life care. We will endeavour to ensure the wishes of an individual and immediate family are respected at all times. We encourage patients to discuss their hopes and wishes with their partner and immediate family and caring professionals as and when appropriate.

Ambulatory Blood Pressure Machine

This device is usually worn for approximately 8 hours, whilst undertaking your normal activities, to accurately determine your blood pressure. They are especially helpful if visits to the surgery provoke anxiety. You will be required to attend the surgery for fitting and then removal after 8 hours.

Childhood Immunisations

We offer Childhood Immunisations based on the NHS vaccine shedule.

NHS screening

The NHS offers screening, free of charge, to identify pre-existing health problems of which you may not be aware, nor be exhibiting any symptoms. The NHS screening may be as part of a national programme for specific demographic groups or may be opportunistic screening when a patient discusses a related problem with their GP.

NHS Health Check aged 40 – 74

The NHS Health Check is a health check-up for adults in England aged 40-74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk. Please contact the Practice to arrange an appointment.
