Should you wish to register with the practice we currently have an open list and will be happy to register you if you are moving into the practice boundary area. Please see boundary map below for details of the areas we cover (use the ‘postcode’ facility on this map to see if you are within our boundary) or call the specific surgery for further details if required.

Our aim is to administer medical care to all patients who are registered with the practice. As you will see from the website we offer a variety of special clinics and provide links to a number of topics including medical conditions, self-help etc.
As a new patient, we would ask that you complete all the forms provided fully as this gives us the opportunity to find a little bit about your medical background. We would also ask that you return to the surgery a week after registering with us to use our ‘Pod’ to complete a New Patient Check to enable us to complete your registration fully.”
We operate an appointment system where we will offer an appointment with the next available healthcare professional as soon as possible, although this will depend on whether you require urgent or routine care.
We recognise that waiting to see a specific clinician can sometimes result in delays in being seen; however we feel that this is the best approach under the current nationally directed guidelines.
Please remember that if you need to see the doctor on an urgent basis then we will always try to accommodate you, however do not be offended if the Receptionist asks you when the problem started and the nature of the problem. They have been instructed to do so by the doctors and it is not meant to be offensive in any way, it just helps us to help you. You are able to pre-book routine appointments with other members of staff i.e. Practice Nurses and Healthcare Assistants.

Prior to registration, to help with our registration process, it is preferable for you to provide two forms of identification; one proof of photo ID and one proof of residency. Please note that your medical records may take several weeks to arrive at the Practice.
Although anyone can register without needing proof of ID, address or immigration status.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
We have the right to refuse to register patients if they have a violent flag against their record.

Dorset Care Record
At present, health and social care organisations in Dorset may hold different sets of records about you. Information in different records may be duplicated or incomplete. The Dorset Care Record is a new confidential computer record that will, over time, include a range of health and social care shared information to help improve the care you receive.
Sharing appropriate information electronically to a single place, will offer direct access for authorised health and social care professionals to provide as full a picture as possible of your history, needs, support and service contacts. It will join records and systems together for the first time in Dorset. In the longer term, the aim is to offer you access to your record.
