Travel Vaccination Assessment Form

We are unable to arrange travel consultations at short notice. Please submit your travel request as early as possible, at least 4 weeks in advance. It may not be possible to vaccinate last-minute travellers and we may suggest that you visit an alternative private clinic.

Some travel vaccines are ordered on a private prescription which incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Please see price list below.

Most travel vaccinations are not supplied by the NHS. If the nurse has assessed that you require vaccinations that are chargeable, you will be asked to pay for your vaccines in full, at your first appointment.
 

Payments can be made in the form of cash or card payment.
