What We Check
During your review, we perform several vital checks:
- HbA1c Blood Test: This measures your average blood sugar levels over the last few months.
- Blood Pressure: Keeping this controlled is important for heart health.
- Foot Check: Diabetes can damage nerves and blood supply to the feet. We check them annually to prevent complications.
- Kidney Function: We test your urine to make sure your kidneys are working well.
The Two-Stage Process
To make sure we have all the results ready for a meaningful discussion, your Annual Review happens in two stages:
Stage 1: The Data Collection (with a Healthcare Assistant)
- What happens: Blood sample taken, blood pressure checked, height and weight measured, and a foot check performed.
- Important: You must bring a urine sample to this appointment.
Stage 2: The Review (with a Diabetic Nurse)
- What happens: A few days later, once your results are back, you will have a consultation to discuss them and create your care plan.
- Action: You need to contact us to book this second appointment after your first visit.
- Note: If your diabetes is not well controlled, we may invite you for reviews more frequently than once a year.
