Key Information:

  • Your “Boarding Pass”: To see you faster and keep you safe, many clinics are now run by highly skilled Healthcare Assistants. Think of them as “Skilled Drivers” on a journey. They can get you to your destination safely, but they cannot change the route set by the “Route Planner” (a Doctor or Nurse).
  • Why the Consent Form is Needed: Because the Healthcare Assistant cannot authorise new treatments on the spot, they need to see a valid “Boarding Pass” before they begin. The consent form is that pass. It proves a clinician has checked your records and authorised the procedure.
  • Your Responsibility: Before you complete the form, you must be sure you understand what you are agreeing to.

The 5 “Pre-Boarding” Checks

Before signing, please ask yourself these five questions. If the answer to any of them is “No” or “I don’t know,” please read the specific information leaflet provided for your procedure.

  1. The Destination (Target): Do you know exactly what this procedure is for (eg flu vaccine, shingles vaccine, B12 injection)?
  2. Reason for Travel (Benefit): Do you understand why we are recommending this for you?
  3. Bumps in the Road (Risks): Are you aware of the common side effects? (Please check the specific information link below for details on your specific procedure).
  4. Staying at Home (Alternatives): Do you know that you can say no? You must weigh the choice of saying no against the health risks of not having the procedure.
  5. The Journey (The Procedure): Do you know exactly what the procedure involves?

What if I am Unsure?

  • Do NOT Call Reception: Receptionists are not clinicians and cannot answer medical questions about risks or side effects.
  • Read the Information: First, read the specific leaflet or link provided for your procedure.
  • Still Unsure?: If you have read the information and still have questions, attend your appointment but DO NOT complete the consent form yet. Instead, ask the clinician in the room to explain before they begin.
  • If You Are Happy: If you are happy with all five checks, please complete the form. This signals you are ready for the journey.
