Patients travelling to our Poole Surgery are advised to allow extra time for their journey due to a road closure in the area from 31 August for 13 weeks.
This health check is not a box-ticking exercise. It is a chance to understand your health better, talk about anything that may be affecting you and see whether extra support could help.
You do not need to be unwell. The check can help us spot small problems early so that they can be dealt with sooner.
People can feel differently about the words “learning disability”. Whatever words feel right to you, the appointment is about making sure you receive the care and support that is right for you.
One of our clinicians will carry out some simple health checks. We may check your weight, blood pressure and pulse.
We may ask whether you are happy to provide a small urine sample or have a blood test, but only if that feels okay for you.
A large part of the appointment is talking about your health, daily life, medicines and anything that has been worrying you.
We can agree on any next steps together and write them in a health action plan to help you stay well.
The appointment is your choice, and you are in control of what happens.
We will explain things clearly and ask for your permission. You can say no to anything you do not want.

This could include a longer appointment, a quieter time of day, simpler words, pictures or bringing someone with you.
